To support the work of our general practice network Pinnacle MHN also directly delivers some services in the community. Your GP or practice nurse will refer you to any services you are eligible for or may benefit from. Services delivered directly by Pinnacle MHN are described below so you can get to know a little more before you may encounter us in your community.
Stop Smoking Service – Once and For All
Our Once and For All programme is available for those in the Waikato and Tairāwhiti regions. The programme combines behavioural support from a 'quit coach' combined with nicotine replacement therapy (NRT) if appropriate. Learn more on the Once and For All website.Extended care teams
Our extended care teams work alongside general practices, focussing on better integration within communities to support long-term condition management plans and child health. Teams include health professionals such as clinical pharmacists, peer support workers, community health workers, dietitians, exercise consultants and more.Support to screening
In the Waikato we offer screening support services for BreastScreen Aotearoa and the National Cervical Screening Programme. We provide targeted follow up, outreach and support to priority group women who experience barriers to accessing breast screening, cervical screening, colposcopy, and assessment and treatment services.Mobile immunisation service
In the Waikato we have a team who help to achieve immunisation targets by following up families/whānau who have children that have not accessed health services for immunisations, or who have missed immunisations. The team also actively work to reconnect families/whānau engagement with their general practice to make sure no other health checks or services are missed.School-based health services
Waikato DHB contracts us to provide school-based health services in high schools, wharekura, teen parent units and alternative education sites. The decile rating of a school determines what services the students are eligible for. If the school your child attends has a school clinic they will provide you with information about the service. School clinics remove some of the barriers that prevent young people from accessing health services, like cost, transport and fear of talking to a health professional. They also provide a way to encourage youth to stay engaged in the health system once they leave secondary school.Coach-Net, a name you already know and trust, is changing the name of our dispatch operations to Pinnacle Motor Club, Inc.
